The Lincolnshire Care Portal (or Lincolnshire Shared Care Record (ShCR)) is a secure computer system that allows health and care staff to access shared patient information with one login and one patient search. This includes a Care Plan capability that enables health and social care professionals to input responses to a pre-formatted Care Plan template that the service owns.
The Lincolnshire Care Portal brings together selected patient information from organisation’s source systems in real time. Below summarises the patient data currently accessible (subject to appropriate Role Based Access Controls) in the Shared Care Record and the owning organisations.
The Lincolnshire Care Portal roll out commenced in 2016 and it is accessible by staff employed by GP Practices, PCNs, ICS, ULHT, LCHS, LPFT, LCC Adult Social Care and St Barnabas.
Getting Access to the Lincolnshire Care Portal
All GP Practice and PCN employed staff are able to access the Care Portal and there are defined processes in place that have been communicated. If you are unsure speak to your Practice Manager or PCN Lead. If you don’t recall seeing these processes please contact firstname.lastname@example.org
